Instruments for Quality Nursing Care in Delirium - Systematic Literature Review

Abstract


Situation - Problem: This study aims to learn Instruments used by Nurses For Nursing Diagnosis: Acute Confusion Risk, Specified hair phenomenon of Delirium . Question clinic: For highly complex client with Acute Mental Confusion diagnosis, which the Delirium Rating Scale most effective for their identification by nurses. Methodology: For this, we performed a search Computed Bibliographic in BVS Portals and PubMed, in LILACS , MEDLINE , CINAHL databases , and these selected Were the ONLY indexed in Journal of the already mentioned databases, not Period 2009 to 2014. Responding to the clinical question: The search found 255 articles using the key words: Delirium/Delirium/Delirio, Intensive Care/Intensive Care, Nursing/Nursing/Enfermería, with Boolean operators and and or, of these 49 were viable for analysis. After speed reading , 10 journals were selected for the study in screen, able to answer the clinical question. Seven scales were found documented for assessment of delirium, three studies did own quizzes. Analyzing the scales, it was found that the CAM -ICU is an important tool in detecting delirium and their use for the team results in a more effective control of severe patients with the disorder as well as those having risk to develop it does. Conclusion: We conclude that monitoring of delirium in the ICU is not only important as an indicator of organ dysfunction , but also to prevent accidental injury. Thus, promotes the reduction of adverse effects and allows the establishment of preventive and therapeutic measures to provide adequate rehabilitation and potentially lower losses related to quality of life . Recommendation for practice: So it is necessary investment in research and training for implementation of protocols and standardization of assessment scales for early detection of delirium .
